Question:

I have a 4-year-old
daughter with a woman I lived with but never married. My name was never put on
the birth certificate. We have since split up. I have been giving financial
support and have had visitation. The mother is unstable and constantly
threatening to take my daughter and telling me that I will never see her again.
I need to know how to have my name put on the birth cert. and arrange
visitation
with my daughter.

order preventing her from doing so needs to be addressed quickly with your
